73세 여자 환자가 항문출혈을 주소로 내원하였다. Anal verge 7cm 상방에 궤양을 동반한 종괴가 만져졌다. 복부 CT상 간 우엽에 전이성 고립 종괴가 있었다. AFP 수치는 정상이었다. 이 환자에게 필요한 치료는?

 73세 여자 환자가 항문출혈을 주소로 내원하였다. Anal verge 7cm 상방에 궤양을 동반한 종괴가 만져졌다. 복부 CT상 간 우엽에 전이성 고립 종괴가 있었다. AFP 수치는 정상이었다. 이 환자에게 필요한 치료는?  1. 저위전방절제술(lower anterior resection) + 간 종괴 절제 2. 항암방사선요법 3. 방사선요법 4. 동맥화학색전술(TACE) 5. 경항문 국소 절제술 - 해설:  AV 상방 7cm 위치에 직장암이 확인되었고, 간 우엽에 전이 병변도 확인되었다.   Metastatic colorectal cancer의 경우 resectable disease라면 원발 병변 절제와 간절제를 시행하는 것이 궁극적인 치료이지만, upfront surgery를 우선 시행하는 colon cancer와 달리,  rectal cancer의 경우 neoadjuvant chemotherapy + radiotherapy (TNT approach)를 우선 시행하고, restaging을 한 후 절제를 위한 수술을 시행 한다.  만약 neoadjuvant therapy 후 complete clinical response가 확인된다면 수술 없이 추적관찰을 시행해볼 수 있다.   (증상이 있는 Obstructing lesion 경우라면 초반에 diversion혹은 resection을 고려할 수 있다)      Recommendations for Treatment of Resectable Synchronous Metastases      When patients present with rectal cancer and synchronous liver- or lung only metastases,      the panel now recommends a TNT approach     , with choice of preoperative therapy based on the predicted status of the CRM by MRI.      Upfront systemic treatment has the goal of early eradication of micrometastases, whereas the goal of short-course RT or long-course chemoRT is local control of disease prior to surgery/local therapy.           참고* 2013년도 NCCN rectal cancer guideline에서는 Resectable synchronous metastases에서 upfront surgery, neoadjuvant CCRT or neoadjuvant Chemo 모두 가능했으나, 2014년도부터는 preoperative therpay를 대부분의 환자에서 시행할 것을 권고하며 upfront surgery 의 option 지워졌다.     2023 NCCN guideline에서부터 locally advanced rectal cancer 에 있어서 TNT approach (Total neoadjuvant therapy)가 우선 권고되고 있다.   기존 StageII, III에서  수술 전 neoadjuvant CCRT  (Capecitabine or 5-FU + RT)를 시행해왔다면,  TNT approach는 수술전에 CCRT (Capecitabine or 5-FU + RT)  와 함께 chemotherapy (FOLFOX or CAPEOX) 를 시행하는 방식이다. 기존의 neoadjuvant CCRT에 한번 더 chemotherapy를 시행하는 방식의 TNT는 locally advanced rectal cancer에서 재발 및 원격 전이의 비율을 낮추고 생존율 향상을 보였다.

해설 수정되었습니다. 첨부하신 사진은 2023 NCCN rectal cancer guideline으로, 2024 guideline에서는 CRM에 대한 구분 없이 치료법이 제시되고 있습니다.참고로 2013년도까지는 resectable liver metastasis가 동반된 rectal cancer에서 neoadjuvant chemo, neoadjuvant CCRT, upfront surgery (rectal lesion resection &amp; metastectomy)가 모두 가능했으나,2014년도부터는 surgical option이 배제되었습니다.<editable-image src="https://bzxuobvwdrvtfqhmvczv.supabase.co/storage/v1/object/public/images/24407" elements="[]"></editable-image><editable-image src="https://bzxuobvwdrvtfqhmvczv.supabase.co/storage/v1/object/public/images/24408" elements="[]"></editable-image>“In the 2014 version of these guidelines, the panel removed the option of surgery as the initial treatment because they believe that the majority of patients should receive preoperative therapy”<editable-image src="https://bzxuobvwdrvtfqhmvczv.supabase.co/storage/v1/object/public/images/24409" elements="[]"></editable-image>
