In euvolemic hyponatremia, the treatment of choice depends on the cause. If it is a deficiency in cortisol or thyroxine, gradual replacement usually suffices to eliminate all signs and symptoms, including the hyponatremia. In SIADH, the excess body water should be eliminated. If the hyponatremia is mild and largely asymptomatic, restricting total water intake to ~30 mL/kg per day less than urine output for several days or until the syndrome remits spontaneously will usually suffice. If, however, the hyponatremia is severe and symptomatic, the goal should be to partially correct it by intravenous infusion of hypertonic (3%) saline or administration of an AVP antagonist such as tolvaptan or conivaptan (Fig. 381-7).
