임신 15주 31세 여성이 질출혈로 내원했다. 자궁경부는 닫혀있고, 니트라진 검사에서는 노란색이였다. 다른 통증, 자궁수축은 없었다. 산모 혈액형은 O형, Rh(-)이다. 적절한 처치는?

 임신 15주 31세 여성이 질출혈로 내원했다. 자궁경부는 닫혀있고, 니트라진 검사에서는 노란색이였다. 다른 통증, 자궁수축은 없었다. 산모 혈액형은 O형, Rh(-)이다. 적절한 처치는?  1. 경과 관찰 2. Rhogam 투여 3. 혈중 b-hCG 측정 4. 흡입 소파술 5. methotrexate 투여 - 해설:  임신  20주 미만에서 질출혈 을 보이고 있는 환자로,  자궁경부가 닫혀있고 양막파수는 없으며  자궁수축도 없는 상태이다.  절박유산 이 의심된다.  대개 절박유산에서는 절대안정 및 경과관찰을 시행하나, 현재 산모가 Rh(-)이므로 감작을 예방하기 위한 예방적 RhoGAM (Rh-D 면역글로불린) 투여가 필요하다.   감작되지 않은 Rh(-) 여성에서 예방적 RhoGAM 투여가 필요한 상황은 다음과 같다.     유산, 인공중절, 자궁외임신, 사산    양수천자, 융모막생검, 제대혈천자    포상기태 긁어냄술    산전출혈 (절박유산 포함)    질식분만, 제왕절개, 태아외회전술    태반조기박리    임신 중 복부 외상
