정의 및 병태생리
- 동의어: Drug-induced Hypersensitivity Syndrome (DIHS)
- 기전: Type IV Severe Cutaneous Adverse Reaction (SCARS), 구체적으로 Type IVb (T2) 반응
- 핵심 기전: T lymphocyte-mediated eosinophil inflammation
- 바이러스 재활성화: HHV-6, HHV-7, EBV, CMV 등의 잠복 바이러스 재활성화와 관련
- 유전적 소인 (HLA): Allopurinol (HLA-B*58:01), Abacavir (HLA-B*57:01), Vancomycin (HLA-A*32:01)
원인 약물
- 고위험군: 방향족 항경련제(Phenytoin, Carbamazepine, Phenobarbital), Allopurinol, Sulfonamides, Minocycline, Vancomycin 등
임상양상
- 잠복기 (Timing): 약물 노출 후 2~8주 후 발생 (2- to 8-week delay) (다른 약물 발진보다 늦게 나타남)
- 전신 증상: Fever (38°C 이상), 권태감
- 피부 증상: Morbilliform eruption(홍역 모양 발진)이 얼굴, 상체, 팔다리로 퍼짐, Epidermal edema (주로 Facial edema), 수포/농포 동반 가능
- 림프절: Lymphadenopathy (흔함)
- 장기 침범: Infiltration of skin and internal organs
  - 간(Hepatitis, 가장 흔함), 신장(Nephritis), 폐(Pneumonitis), 심장(Myocarditis) 등
진단 및 검사 소견 (Biomarkers)
- 혈액 검사:
  - Eosinophilia (>1,000/µL)
  - Atypical lymphocytosis
  - 간효소 수치 상승, 신기능 저하
- 사이토카인 및 케모카인 (Figure 63-1):
  - Interleukin-4 (IL-4) 증가
  - Interleukin-5 (IL-5) 증가
  - Eotaxin 증가
- 진단 기준: RegiSCAR 점수 등 활용
치료 및 관리
- 1차 조치: 즉시 원인 약물 중단 (Avoid medication)
- 금기 사항: Desensitization not indicated (탈감작 요법 금지)
- 약물 치료:
  - 중등도~중증: Systemic steroid (Prednisone 1.5~2 mg/kg/day equivalent)
  - Tapering: 재발 방지를 위해 8~12주에 걸쳐 매우 서서히 감량
  - Steroid-sparing agents: 스테로이드 감량 중 재발하거나 반응이 없을 때 IVIG, Cyclosporine, Mycophenolate mofetil 고려
- 모니터링: 심근염(Myocarditis) 등 지연된 장기 손상 확인을 위해 장기간 추적 관찰 필요
