그외 다른 기흉들은 긴장성 기흉처럼 심하기 않을 것이기에 기관지가 정상측 폐로 편위되는 것은 아니라는 것이지, 병측으로 편위되는 것은 안될 거 같습니다.

제가 알기론 긴장성 기흉에서 기관지 편위가 정상측으로 편위되는 이유로는 기흉으로 인해 음압을 유지하던 폐공간에 공기가 들어오면서 양압으로 바뀌면서 이로 인해 기관 편위가 정상측으로 밀리는 것으로 보입니다.

Pneumothorax radiographic features

A pneumothorax is, when looked for, usually easily appreciated on erect chest radiographs. Typically they demonstrate:

• visible visceral pleural edge is seen as a very thin, sharp white line

• no lung markings are seen peripheral to this line

• peripheral space is radiolucent compared to the adjacent lung

• lung may completely collapse

• mediastinum should not shift away from the pneumothorax unless a tension pneumothorax is present (discussed separately)

• subcutaneous emphysema and pneumomediastinum may also be present

Tension Pneumothorax radiographic features

A tension pneumothorax will have the same features as a simple pneumothorax with a number of additional features, helpful in identifying tension. These additional signs indicate hyperexpansion of the hemithorax:

• ipsilateral increased intercostal spaces

• contralateral shift of the mediastinum (즉 병변측 반대이니 정상측 편위)

• depression of the hemidiaphragm

In the rare instance of bilateral tension pneumothoraces, there may be no cardiomediastinal shift ^6,7. 

19년도 kmle 4교시 25번의 경우 기흉이 일어난 쪽으로 기관이 편위된 것으로 보입니다…!

영상의학과 전공의 선생님께 여쭤본 결과, 원래 성인은 aorta때문에 약간 오른쪽으로 치우쳐보인다고 합니디. 현재 사진은 거의 normal range에 가깝다고 설명해주십니다.
