초음파에서는 uniloculated, anechoic cyst 및 cyst 뒷편으로 posterior enhancement가 관찰되며 이는 simple cyst (주로 follicular cyst)에서 나타나는 소견입니다.
자궁내막종의 경우 초음파에서 cyst 내부에 diffuse homogenous echogenicity (ground glass opacity)가 보이는 소견이 특징적입니다.

Ovarian cyst의 경우 초음파 검사로 그 종류를 어느정도 감별할 수 있습니다. 전부 설명하기는 어려우나, 특징적인 소견 몇개만 정리해두겠습니다.

FUNCTIONAL CYSTS (non-neoplastic)

• [Simple cyst /follicular cyst]

Uniloculated, thin walled, anechoic clear fluid,
no internal wall irregularity (no septations / loculation / papillary projections)
내부에 어떤 음영도 없이 완전히 까만 (anechoic) 소견을 보이며, cyst 뒷부분으로 아주 밝은 영역이 보이는데 이는 낭종이 clear fluid 로 차있기 때문에 posterior enhancement /enhanced through transmission 가 발생하는 것 (빨간색)

Functional cyst이기 때문에 시간이 지나면 저절로 사라짐

• [Hemorrhagic cyst / corpus luteum cyst]

Uniloculated cyst, internal reticular spider web pattern (clot & fibrin strands)
“ring of fire” vascular pattern around the cyst

• [Theca lutein cyst]

  Bilateral, multiloculated cyst. B-hCG 자극에 의한 benign, functional ovarian enlargement로, 주로 임신, h.mole, multi-fetal pregnancy, ovarian hyperstimulation 등에서 보임. 합병증이 발생하지 않는 이상 특별한 치료를 요하지 않음.
  

  

OVARIAN CYSTIC NEOPLASMS (neoplastic tumors, benign-borderline-malignant 가능)

• [Endometrioma]

Unilocular/multilocular (<5 locules) tumor, diffuse low level echogenicity representing old blood in cyst cavity - “ground class opacity”

• [Dermoid cyst]

다양한 영상 소견
Rokitansky nodule with posterior acoustic shadowing (1)
/ acoustic shadows & bright echoes representing hair in cyst (2)
/ intracystic fat balls (3) / internal calcification, fat 등이 포함된 heterogenous mass (4)

• [Serous cystadenoma]

Unilocular or multilocular, smooth thin walled, anechoic cyst
mean size 5-8cm, fine septation이나 internal hemorrhage (echogenic areas) 동반될 수 있음
