• 악안면 골절이나 skull fracture의 가능성이 있는 환자의 경우 비강을 통한 tube insertion이 금기에 해당합니다. Cribriform plate 를 뚫고 intracranial penetration 될 위험이 있기 때문에 대신 orogastric route를 통해 삽입할 것을 권고합니다. (마찬가지 이유로 nasotracheal insertion은 금기이고, oropharyngeal intubation만을 시행합니다.)

  
  

  If a patient has severe facial fractures or possible basilar skull fracture, insert the gastric tube through the mouth to prevent passage of the nasal tube through the cribriform plate into the brain.

  Facial, frontal sinus, basilar skull and cribriform plate fractures are relative contraindications to nasotracheal intubation. Evidence of nasal fracture, raccoon eyes, Battle’s sign, and possible cerebrospinal fluid leaks are all signs of these injuries. As with orotracheal intubation, take precautions to restrict cervical spinal motion.
